ARTICLE 1 - VOLUNTEER SERVICES:
Volunteer will be donating time to the organization in the capacity of the following: VOLUNTEER'S TITLE (the "Role").

In the Role, Volunteer will be providing the following services (the "Volunteering Services"):

Support the Organization’s goals and objectives and do everything in your power to promote and achieve them

Participate in all orientation and training programs

Only undertake duties that you are authorized to carry out and always operate under the direction and supervision of nominated staff and obey reasonable directions and instructions

Understand and comply with the Organization's policies and procedures

Behave appropriately and courteously to all staff, students, and any other party or parties the Organization has relations with

Be open and honest in all dealings with the Organization

Notify the Organization if you wish to change the nature of your contribution

Comply with local laws at all times

Any other duties as assigned

Volunteer plans to volunteer in the Role for ___As needed___(HOURS) hours each week. The Volunteer's specific schedule will be as follows:

ARTICLE 2 - NO WAGES:
Volunteer understands that this role is without wages, a salary, or other benefits. This is an exclusively voluntary role so that Volunteer may provide assistance to the Organization.

ARTICLE 3 - REIMBURSEMENT:
Volunteer may be reimbursed for the following expenses:

ARTICLE 4 - RELATIONSHIP:
Nothing herein shall be taken to establish any partnership, joint venture, agency, or employment relationship. Volunteer is, and at all times will be, a volunteer only. Volunteer shall receive no benefits from Organization, such as health insurance, paid vacation, or sick days. Volunteer shall be responsible for all of Volunteer's own taxes.

ARTICLE 5 - SUPERVISOR:
Volunteer's direct supervisor will be the following: Master Jae Hyeon Jeon (the "Supervisor"). Volunteer agrees to follow all directions given by the Supervisor, as well as any and all rules and instructions provided by Organization. Volunteer may also be assigned other supervisors, including other volunteers, from time-to-time.

ARTICLE 6 - TRAINING:
Volunteer may be asked to undergo training for the Organization. Volunteer understands that such training may be an essential component of being able to volunteer in the Role. Volunteer agrees to complete such training prior to beginning in the Role with Organization.

ARTICLE 7 - MEDIA AGREEMENT
At the discretion of the Organization’s management, any violation of the expectations outlined below may lead to the termination of your volunteer position with the Organization.

Volunteer agrees to maintain the confidential information of the organization.

Volunteer understands that all activities and reporting conducted while volunteering is associated with the Organization and must be mentioned and credited in all media presentations, including but not specific to written articles, videos, television, film, or email blasts.

Any publicity in association with this or any other projects must be approved directly through the Organization. Volunteer agrees to not speak with the media unless authorized first by the Organization.

Volunteer agrees to provide the Organization with a copy of any pictures or videos taken at the Organization and/or any organizational related events.

Volunteer agrees not to post any photos, videos or writings on the internet that are Organization-related (including Facebook, personal blogs). This does not include sharing or linking media content that originated from the Organization (e.g. sharing the Organization’s Facebook post/photo).

ARTICLE 7 - NON-DISCLOSURE AGREEMENT

CONFIDENTIAL INFORMATION: All data, materials, knowledge, and proprietary information generated through, originating from or having to do with the Volunteer program or persons associated with its activities is to be considered Confidential Information and is not to be disclosed to any outside party. This includes, but is not limited to, documents, information, designs, printed matter, policies, procedures, conversations, messages (received or transmitted), resources, contacts, email lists, and email messages, whether internally between staff or outside the Volunteer Program is confidential and the sole property of the Organization’s Volunteer program.

CLIENTS: Client information, including all file information, is not to be disclosed to any third party under any circumstances without the written consent of the Organization.

DAMAGES: Any disclosure, misuse, copying or transmitting of any material, data, or information, whether intentional or unintentional, will subject Volunteer to disciplinary action, prosecution, and/or monetary damages according to the procedures set by the Organization and any applicable laws.

ARTICLE 8 - INDEMNITY:
Organization hereby agrees to indemnify Volunteer against any and all damage, liability, and loss, as well as legal fees and costs incurred, as a result of any act or omission of Organization, Organization's employees, or Organization's agents related to the Volunteering Services rendered under this Agreement.

Volunteer hereby agrees to indemnify the Organization against any and all damage, liability, and loss as a result of any act or omission of Volunteer related to the Volunteering Services rendered under this Agreement.

Article 9 - RELEASE OF LIABILITY:
Volunteer hereby releases and discharges Organization and Organization assigns, successors, officers, employees, agents, executors, partners, administrators or any other legal representatives along with anyone claiming through them (hereinafter, collectively, the "Released Parties"), in their individual capacity or in their business capacities, of all claims, causes of actions, liabilities, disputes, demands, damages, agreements, contracts, obligations, promises, debts, and/or accounts of any kind or any nature, whether currently known or unknown, for any damage, loss, or injury of or relating to the Volunteering Services which Volunteer has or ever had or may have in the future against Organization or any of the Released Parties.

ARTICLE 10 - TERMINATION:
This Agreement may be terminated by either party at any time.

ARTICLE 12 - GENERAL PROVISIONS:

GOVERNING LAW: This Agreement shall be governed in all respects by the laws of the state of Illinois and any applicable federal law. Both Parties consent to jurisdiction under the state and federal courts within the state of Illinois. The Parties agree that this choice of law, venue, and jurisdiction provision is not permissive, but rather mandatory in nature.

LANGUAGE: All communications made or notices given pursuant to this Agreement shall be in the English language.

ENTIRE AGREEMENT: This Agreement constitutes the entire agreement between the Parties and supersedes any prior or contemporaneous understandings, whether written or oral.

HEADINGS: Headings to this Agreement are for convenience only and shall not be construed to limit or otherwise affect the terms of this Agreement.

In consideration of being allowed to participate in any way in SUNRISE TAEKWONDO & AMERICAN MARTIAL ARTS ASSOCIATION athletic sports program, related events and activities, the undersigned acknowledges, appreciates, and agrees that:
1. The risks of injury and illness (ex: communicable diseases such as MRSA, influenza, and COVID-19) from the activities involved in this program are significant, including the potential for permanent paralysis and death, and while particular rules, equipment, and personal discipline may reduce these risks, the risks of serious injury and illness do exist; and,
2. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and,
3. I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; and,
4. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS SUNRISE TAEKWONDO & AMERICAN MARTIAL ARTS ASSOCIATION their officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event (“RELEASEES”), WITH RESPECT TO ANY AND ALL INJURY, ILLNESS, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law.

I grant consent and permission to SUNRISE TAEKWONDO & AMERICAN MARTIAL ARTS ASSOCIATION and its agents and employees the irrevocable and unrestricted right to reproduce the photographs and/or video images taken of me, or members of my family, for the purpose of publication, promotion, illustration, advertising, or trade, in any manner or in any medium.

I hereby give my consent to SUNRISE TAEKWONDO & AMERICAN MARTIAL ARTS ASSOCIATION and its affiliates and agents, to use my image and likeness and/or any interview statements from me in its publications, advertising or other media activities (including the Internet). This consent includes, but is not limited to:

(a) Permission to interview, film, photograph, tape, or otherwise make a video reproduction of me and/or record my voice;
(b) Permission to use my name;
(c) Permission to use my videos, photographs, tapes or video productions submitted; and
(d) Permission to use my statements or quotes from the interview(s) (or excerpts of such quotes), the film, photograph(s), tape(s) or reproduction(s) of me, and/or recording of my voice, in part or in whole, in its publications, in newspapers, magazines and other print media, on television, radio, on the website and/or affiliates websites, other electronic media (including the Internet), in theatrical media and/or in mailings for educational and awareness without restriction.

I hereby waive any right to inspect or approve the finished photographs or electronic matter that may be used in conjunction with them now or in the future, whether that use is known to me or unknown.

I hereby release SUNRISE TAEKWONDO & AMERICAN MARTIAL ARTS ASSOCIATION and its legal representatives for all claims and liability relating to said images or video. FREQUENTLY ASKED QUESTIONS

Q: What is this event about?
A: The International Virtual TKD Expo is being hosted by American Martial Arts Association. The goal of this event is to inspire our students, support our schools, and celebrate the Martial Arts community during this time of crisis.

Q: When is this event going to be held?
A: The registration opened now and it ends on Dec. 20^th 2020

Q: Can anyone participate in this event?
A: YES. It is open to ALL Martial Arts students from any belt level, discipline, school affiliation or experience.

Q: How much does it cost to participate?
A: It is $40 for one event and $10 for each additional event. Entry fees must be paid at the time of registration. There will be no refunds issued after a student’s registration is complete.

Q: What events are there to participate in?
A: There are a total of 6 events. Five (5) individual events and one (1) Demo Team event. The event categories are Official traditional form that is recognized by the Martial Art discipline; creative form with or without weapons; fitness challenge 1 (combo kicks); fitness challenge 2 (triathlon of pushups, situps, and squats), Grand Championship and Demo Team. The Grand Championship event can be participated either as an individual or as a family.

Q: How do I participate?
A: Participants will register online using our website and complete all the necessary waivers. After their registration is received, participants will be invited to a link where they will be able to upload their video submission. Information and training aids will also be added and available on the Facebook page. Each competitor will upload their submission video using the closed FB group at the time of registration or before the event deadline. The video that is submitted will be designated as the FINAL round except for the Grand Championship and the Demo Team events.

Q: What awards are being presented?
A: Each participant will be awarded a medal as well as an electronic certificate that they can print at home. (excludes Demo Team Event- See demo team section for details on awards)*

Q: When will I receive my award(s)?
A: All awards will be mailed to the respective Dojang within 4-8 weeks except the online electronic certificates. Those can be printed at home immediately after the tournament.

Q: Tell me more about the Grand Championship event?
A: It is an event for both individuals or families where participants are encouraged to be creative and keep a 15 day (minimum) video diary about their journey through their training process. These videos can be short snippets. The final video that is submitted cannot be longer than 5 minutes. The winner of this event can either choose a Home dojang or $1000.00 USD (prize money can only be paid once the event has reached 500 participants).

Q: Tell me more about the Demo Team Event?
A: The videos for submission for this event can be either from a current or past performance and cannot be longer than 10 minutes. The winner of this event will receive $1000.00 USD (prize money can only be paid once the event has reached 500 participants).

Q: How will you determine the winners for the Grand Championship and Demo Team events?
A: The top 8 videos selected will advance to the FINAL round. In the FINAL round, the winning video will be determined by the number of likes it receives on our channels.

Q: What is the Dojang CARE Initiative?
A: American Martial Arts Association is providing an opportunity for each school or organization to receive a monetary incentive of up to $20,000 USD based on the level of participation. This incentive is designed to help you create a campaign to grow support and participation by creating groups with other schools to increase your overall number of students registered. These groups will also help you build a community in your local area or state that will establish bonds within the Taekwondo community.

Q: How are the participation numbers calculated for the schools?
A: Only participants registered in individual events will qualify towards your team student totals. (PLEASE NOTE: if a student is registered in the Demo event and not in an individual event, they will not be counted).

Q: How can I register for the Dojang CARE Initiative portion of the event?
A: To take part in the Dojang Cares portion of the event, school owners must verify that they are the owner/operators of the school applying and must fill out a separate verification form to participate. All monies will be paid directly to the school listed on the students’ registration application (not the organization).

Q: Can different schools get together and create a team or organization to increase their participation numbers?
A: YES. To participate as part of a group, each student must know the name of their group and fill out that portion of their registration correctly. Your Dojang name should be listed as the school name, and the group name would go under the group/organization name. Please note: Failure to fill this out correctly will result in a miscalculation of the number of participants, and monies will not be awarded.

Q: How are the monies going to be paid if my school joins together with another school?
A: The monies will be paid to the school the student is registered under to ensure that funds are distributed as timely and efficiently as possible.
